Do you accept my insurance plan?
We do not accept dental insurance plans except for Tricare for military personnel. After your appointment, we will submit to your insurance plan. When we receive payment from them we will send you a bill for your portion of the payment.
I’m a new patient. What should I do to get ready for my first visit?

Click here to print and bring in a completed Medical History Form on your first visit to our office. Also, please bring your insurance card or information (if you do not have a card), and arrive 10-15 minutes before your scheduled appointment time.
I don’t have insurance. Can I make any payment arrangements?
Our relationship with you, our patient, is of primary importance to us. You may be assured that we will always make dental recommendations based solely on what we feel is the best interest of your dental (and overall) health. That being said, we also realize that patients have legitimate concerns associated with payment for their treatment, and we feel that a clear understanding of our mutual obligations and responsibilities is absolutely necessary. We will be happy to discuss fees and financial arrangements at any time, and for your comfort and convenience, we offer a range of options to address payment of fees:
We offer the following options for payment: CASH, CHECK, CREDIT CARD (Visa, MasterCard or Discover).
We do offer a 10% discount for patients without dental insurance if total payment is made at the time of service.
Where should I park when I come for my appointment?
There is a municipal parking lot with meters across the street from the office.
